- Obtain Documentation for your Medical Condition
sign a medical release at your Doctor’s office or our office. Our fax number is (520)-300-8484. No fees are charged for faxing of medical records. ATTENTION: PAST 12 MONTHS of medical records are needed for your appointment. All conditions must be preexisting and documented by your established doctor. We do not diagnose medical conditions “on the spot” for new patients.
- Get Recommended
schedule an appointment with one of our Qualified Doctors online or by calling our office (520)-323-0069. They will evaluate and consult with you on your medical condition. If you qualify, a recommendation will be written.
- Get Registered
Once you have been recommended by our Doctor, we will submit your application to ADHS (Arizona Department of Health Services) – the department that facilitates the Medical Marijuana program. This process also includes taking your photo, filling out and uploading all documentation into your application online. ADHS will mail your card once the application has been processed and approved. Patients also have the option to process the application to ADHS on their own.
